...then I probably shouldn't scare you with the results of one very bad bout of insomnia I had a couple of years ago, where I came up with a hypothetical view of Tamaranean physiology that was based on the concept of a species involving dickgirls... yet which was based virtually entirely on things actually seen among mammals in real life, with it definitely NOT actually being a girl with boy parts. (Considering that, in the real world, the penis develops from the clitoris when certain hormones are applied during gestation, it's not that far-fetched... look at the hyena, for example.)

Perhaps the penis is divorced from it's sexual connotation, at least directly. It could simply a physical representation of aggression, removed from context and transposed onto the objects of their desire; those with such a fetish could want the woman to be aggressive in their sexual pursuits, subverting the role normally held by the male.

For most it's probably the simple explanation of getting to look at two girls while also getting to see penetration. Now obviously when it's shemale-on-shemale and so on you're getting into a murkier area, but again, if you're talking about fantasy, why stop at half-measures? (IE the strapons.) Again, easy enough to say "Why not just read stories about cops and firemen? It requires a lot less handwaving than all the big muscular guys in spandex with the power of hitting things really hard."
